Functional Needs Registry
The Functional Needs Registry is a program of City of Lubbock Public Health Preparedness (PHP). The program is for persons living in Lubbock County who may need assistance during a public health emergency. The goal of the Functional Needs Registry is to provide registrants with emergency preparedness education and information about state and local resources.
City of Lubbock Public Health Preparedness (PHP) is responsible for the Registry. Information collected is transferred to a secure, access-restricted, electronic database. It may be shared only with first responders (Fire, Police, EMS) or other emergency management entities to enhance efforts during a public health emergency.
Lubbock County residents may register by completing the Functional Needs Registration Form
- Mailing the form to City of Lubbock PHP, P.O. Box 2000, Lubbock, TX 79457.
- Faxing the form to City of Lubbock PHP at 775-3903.
- Placing the form in designated drop-boxes at Citibus, 801 Texas Avenue, in Lubbock.
- Returning the form to an Alliance for Functional Needs member agency.
PHP and local community organizations formed the Alliance for Functional Needs in 2007 to help identify services available for persons in Lubbock County with functional needs during an emergency.
