The Office of Vital Statistics is located at City Hall, 1625 13th Street, Room 102.

This office is responsible for filing all birth, death, and fetal certificates in Lubbock County.

There are three deputy registrars in this office that can assist customers. They are available to notarize documents pertaining to vital records such as amending birth or death certificates, filing a delayed birth certificate, or filing an Acknowledgement of Paternity.

Since 1992, this office has utilized a project called "Remote Site Access" which is provided by the Texas Department of Health Bureau of Vital Statistics. This project enables our office to serve the citizens of Lubbock County, and other citizens born in other Texas counties, to obtain their birth certificates.

Qualified Applicants: A qualified applicant is defined as the registrant or immediate family member (either by blood or marriage), his or her guardian, or his or her legal representative.

  • Parent
  • Brother or sister (copy of their birth record to verify parent)
  • Legal guardianship (with proof of court order papers)
  • Grandparents (copy of son's or daughter's birth record to verify parent)
  • Step-parents (can obtain a stepchild's birth certificate up to the age of 17 years old with proof of marriage license)

(There is a $9.00 "Search Fee" if requested record is not found.)

Please note: This office has authority to conduct verification of Texas births only.  Birth records are confidential for 75 years and death records for 25 years; therefore, issuance is restricted to qualified applicants. Please attach a photocopy of ID to application. Failure to complete the application properly may result in the application being rejected or delayed in processing.

HOW DO I OBTAIN A BIRTH/DEATH CERTIFICATE?

In order to obtain a Birth Certificate you MUST BE A QUALIFIED APPLICANT & the following information is required:

  • Name of Person on Record.
  • Date of Birth.
  • Parents' name, Mother's Maiden name.
  • County Born.
  • Photo copy of your Driver's License.
  • Self-addressed stamped envelope.
  • Acceptable forms of Payment: Cash, Credit Card, or Money Order. (NO CHECKS)
  • Money Order for $23.00, Made Payable to: City of Lubbock.
  • Phone number where you may be reached between 8am-5pm.

In order to obtain a Death Certificate you MUST BE A QUALIFIED APPLICANT & the following information is required:

  • Name of Person on Record.
  • Date of Death.
  • Parents' name, Mother's Maiden name.
  • Photo copy of your Driver's License.
  • Self-address stamped envelope.
  • Acceptable forms of Payment: Cash, Credit Card, or Money Order. (NO CHECKS)
  • Money Order $21.00 for first copy, each additional copy is $4.00. Made Payable to: City of Lubbock.
